Jack Major aged 53, presented to the emergency department. He has been unwell and experienced right, lower quadrant abdominal pain for 4 days. He had a history of nausea and vomiting over last 2 days. Jack stated that he had episodes of diarrhoea over the past year. He lost weight and felt very fatigued. Jack largely ignored these symptoms as he thought his lifestyle was not healthy – eating lots of takeaway food and being busy with his political activities, university, and social life. Jack reported no significant medical or surgical history and is not taking any medications. He is allergic to peanuts with anaphylactic reactions. Jack was assessed by the MO and found tenderness in lower right abdominal quadrant.

 Jack was admitted to hospital and had the following investigations: 

• abdominal CT scan

 • colonoscopy 

• endoscopy (upper)

 

After substantial testing Jack Major is found to have diverticulitis. After handover you go to check on Jack and find he is complaining of pain abdominal pain 7/10. His saturations are 94%, RR 18. He is vomiting and his abdominal is distended and has a low-grade fever at 37.4, his BP is 95/58 and HR 110. Jack does not have any altered calling criteria ordered.
